What Are the Signs and Symptoms of Angina?
Pain and discomfort are the main symptoms of angina. Angina is often described as pressure, squeezing, burning, or tightness in the chest. It usually starts in the chest behind the breastbone.
Pain from angina also can occur in the arms, shoulders, neck, jaw, throat, or back. It may feel like indigestion.
Some people say that angina discomfort is hard to describe or that they can’t tell exactly where the pain is coming from.

Every Mental Illness is a Chronic Disease: Including Depression
Misconceptions about mental illness have existed for centuries. Accused of everything from moral failings to demonic possession, people with mental illness had been forced to hide their suffering. The Surgeon General of the United States reported in 1999 that stigma was the single largest barrier to the recovery of people with mental illness, making it harder for people to get treatment as well as find housing, jobs, and friends. Nearly two-thirds of people with mental illness do not get the help they need.
Today we understand that mental illness is not only treatable, but that it is a chronic disease like asthma or diabetes. Classifying mental illness as a chronic disease may seem surprising, but consider the statistics. Nearly one in five Americans has a mental disorder, and mental illness is the leading cause of workplace absenteeism. Depression, one of the most common mental illnesses, is harder on people’s health than long-term illnesses like angina, arthritis, asthma, and diabetes. A new study by the Centers for Disease Control and Prevention finds that depression and anxiety are two major causes of health problems and chronic illnesses, including asthma, diabetes, and cardiovascular disease.
Emergency rooms are overcrowded with people with mental illness. According to the Agency for Healthcare Research and Quality, almost one-fourth of all stays in U.S. community hospitals-7.6 million of nearly 32 million stays-involved depression, bipolar disorder, schizophrenia, and other mental health disorders or addiction disorders. Our contemporary response to mental illness has been to treat mental illness as an acute illness-with a hospital stay followed by a referral to a doctor or clinic in the community.
Unfortunately, the outcome is often relapse and repeated cycles of high-cost hospital stays. The coverage for treatment of mental illness contrasts sharply with its chronic nature. Only an estimated one-fifth of U.S. workers with employer sponsored health insurance are covered by strong parity laws that mandate mental health benefits, prohibit limits on outpatient visits and inpatient days, and limit the extent to which enrollees are burdened with higher cost sharing for mental health services.
Managing the Disease
With adequate treatment and support, people can learn to manage their mental illness and can recover sufficiently to have full, productive lives even if they are not cured. In fact, recovery rates for mental illnesses like depression, anxiety, schizophrenia, and bipolar disorder surpass the treatment success rates for many physical disorders such as heart disease.
But there are major challenges to the well being of people with mental illness. Contributing to the complexity of successful treatment is poor employment status and resulting poverty; high levels of substance use and physical illnesses; and difficulty with adherence to treatment regimens.
The same kind of care-management approaches effective in treating physical conditions such as diabetes or asthma-approaches that offer continuity, coordination and comprehensiveness-can also work for mental illness. The millions of Americans who are living with mental illness need the confidence and skills to manage their condition; the most appropriate treatments for optimal disease control and prevention of complications; a mutually understood care plan that includes coordination among all physicians and support-service providers; and careful, continuous follow up.
People with mental illnesses are no different than people living with arthritis, diabetes, and other chronic diseases. They need to be evaluated, insured, and treated. They need continued care and monitoring.
Ending Insurance Discrimination
Adequate care requires adequate dollars. Public insurance pays for at least 75 percent of treatment services for people with chronic and serious mental illnesses in community settings. Despite its discriminatory approach, it is the only option for millions of poor, unemployed, homeless, incarcerated, and other vulnerable populations with mental illness. So we must act to end the discrimination. We must eliminate ongoing and threatened cuts to Medicaid. And Medicare must stop requiring higher copays from people with mental illness, while strengthening its sadly inadequate mental health benefits package.
Private insurance also discriminates against people with mental illness. Currently, only an estimated one-fifth of U.S. workers with employer sponsored health insurance are covered by strong parity laws that mandate mental health benefits, prohibit limits on outpatient visits and inpatient days, and limit the extent to which enrollees are burdened with higher cost sharing for mental health services.
The recent House passage of the parity bill-the Paul Wellstone Mental Health and Addiction Equity Act-offers hope, taking us closer to ending discriminatory private health insurance policies that deny or restrict access to treatment for people who are suffering from mental illnesses and addiction disorders. If it becomes law, the bill will require health insurers to offer mental health benefits equal in cost and scope to medical and surgical benefits. It will prevent insurers from requiring larger copayments or imposing lower reimbursement ceilings for mental illnesses and addictions.
We have indeed come a long way in how we perceive and treat mental illnesses. But more needs to be done to meet the needs and manage the costs of this chronically ill population. Now is the time to put mental illnesses on an equal footing with other chronic diseases. The sooner that mental illnesses achieve parity, the sooner millions of Americans can get on the road to recovery.

Another sign of a heart attack can be a shortness of breath. If you are a very active person, you know what it will feel like when you are unable to catch your breath or take in a deep breath. The signs of a heart attack include not being able to take a breath, or a deep breath which can in turn leave you feeling dizzy and feeling as if you need to sit down for a while. A heart attack symptom is not one that you should ignore. If you are feeling combinations of symptoms it can be vital to get to your doctor as soon as possible.
If you feel pains or discomfort in your arms, legs or in the chest, and you feel as if your neck or jaw are hurting uncontrollably, these could be additional signs of a heart attack. Heart attack symptoms will vary from person to person, but pain that is not normal, that you don’t usually feel, in combination with shortness of breath and or a tightness in your chest could be a huge sign that you are heading towards a heart attack.

I get burning in my chest and other pain.I have alot of gas and have been having panic attacks and anxiety.I do get pains in my arm and sometimes my chest to my chin or teeth.Could this be anxiety or angina?And if it is anxiety does the chest pain stay with you until you get rid of the anxiety because they come and go.Thanks.

However, I can tell you that as a sufferer of panic attacks for more than 4 years, I get chest pains and tingling/numbness in my left arm. These can be signs of a heart attack, but I have come to realize that it happens every time I am about to have a panic attack.

Carole Willi, M.D., of the University of Lausanne, Switzerland, and colleagues conducted a systematic review and analysis of studies describing the association between smoking and the incidence of diabetes or other glucose metabolism irregularities that covered thirty years. The data was published in the Journal of the American Medical Association’s December 12, 2007 issue.
The data indicated that active smokers have a 44 percent increased risk of developing type 2 diabetes compared with nonsmokers. The more you smoke, the greater the risk. People who smoked twenty or more cigarettes a day had a 61 percent increased risk, compared with lighter smokers, who had a 29 percent increased risk.
Death, regular or menthol: Most people associate smoking with cancer. However, its effect on the cardiovascular system and diabetes is stealthy, and your entire organ system can be damaged before you become aware of the extent of the damage. Smoking is an insidious killer. It takes years but by then it is too late.
According to the American Heart Association, more than 440,000 people die each year from smoking-related diseases, and 135,000 additional deaths are linked to the effects of cigarettes on the cardiovascular system. Smokers are two to three times more likely to die of cardiovascular disease than are nonsmokers, according to the American Heart Association.
According to the American Diabetes Association report Smoking and Diabetes, cigarette smoking accounts for one out of every five deaths in the United States and is the most important modifiable cause of premature death. Other studies consistently find heightened risk of morbidity and premature death associated with the development of macrovascular complications among smokers. Smoking is also related to the premature development of microvascular complications of diabetes.
The cardiovascular burden of diabetes, especially in combination with smoking, has not been effectively communicated to people with diabetes or to health-care providers, and there is little evidence that this risk factor was being talked about consistently.” According to researchers at the Cleveland Clinic Heart Center, “There is no safe amount of smoking. Smokers continue to increase their risk of heart attack the longer they smoke. People who smoke a pack of cigarettes a day have more than twice the risk of heart attack than nonsmokers.”
According to the National Institute of Drug Abuse, within 24 hours of quitting, blood pressure and chances of a heart attack decrease. A 35-year-old man who quits smoking will, on average, increase his life expectancy by 5.1 years. The more you smoke, the more likely you are to become atherosclerotic, which results in blocked arteries and reduced blood flow to the heart. If you are diabetic, your diet is poor, and you don’t exercise regularly, every cigarette multiplies your risk of developing angina and coronary artery disease.
The peripheral arteries that carry blood to the arms and legs are at increased risk for blockages in smokers, and smokers may suffer symptoms of intermittent claudication (leg pain and cramping due to impaired blood flow). There is also a greater chance of central artery blockages, increasing your risk of stroke.
In addition to increasing your risk of cancer of the lung, mouth, esophagus, and bladder, smoking raises your likelihood of developing chronic obstructive pulmonary disease, emphysema, and gastrointestinal problems, such as gastroesophogeal reflux disease (GERD) and ulcers.
Here are a few more effects of smoking:
- Smoking causes your blood sugar to rise.
- Cholesterol levels and other lipid levels rise as well.
- Smoking can cause high blood pressure.
- Blood vessels are restricted, which can lead to foot ulcers and leg and foot infections in people with diabetes.
- It increases the chance of thrombosis (blood clots).
- Neuropathy (nerve damage) develops or worsens, leading to sexual dysfunction and kidney damage.
- Your immune system becomes compromised, making you more susceptible to infections, colds, bronchitis, and pneumonia.
- It reduces levels of vitamin C in the body, which help your body develop antioxidants (preventing heart disease and cancer), collagen for stronger bones and cells, and healthier gums and also speeds healing of scrapes and burns.
- You inhale 400 different toxins in the smoke and 43 known carcinogens (i.e., cancer-causing agents) every time you take a drag. These include a tar similar to road surfacing tar, the poisonous gas carbon monoxide, arsenic, formaldehyde, ammonia, and many other poisonous compounds. These chemicals circulate in your body, continually putting you at long-term risk, as myriad scientific studies have proven.
- Women who smoke and use oral contraceptives are at higher risk of coronary and peripheral artery diseases, heart attack, and stroke than are nonsmoking women who use oral contraceptives.
- The likelihood of developing complications from medications increases.
All of this increases your chances of dying before your time and makes weight loss more difficult.

Periapical and Dentoaveolar Abscesses
Abscess make up of about 2% of all dental problems around the root tips of the teeth. This oral infection is divided into two forms. The primary (neoteric) form involves the inflammation of the pulp and has little no significant change that can be seen on the dental x-rays. They also named by our Houston dentists as an acute apical periodontitis or acute periapical abscess. The secondary (recrudescent) form comes from a previously existed, asymptomatic lesions such as granuloma, cyst, scar, and cholesteatoma.
The causes and the course of the disease:
The primary abscess always appears suddenly. It is associated with the root tip of the tooth and a dead pulp. The inside of the root canal contains large numbers of live bacteria that spread quickly into the surrounding tissue. This rapid spread of the infection causes the dental ligament to become inflamed (acute periodontitis) and results in an extremely painful toothache. Sometimes, the inflammation is so sudden and severe that it pushes the tooth slightly out of the tooth socket and brings even more pain when chewing.
The secondary abscess may be either acute or chronic, depending several factors such as the number and the aggressiveness of the offending bacteria, the immune system of the patient, and the type and timing of the treatment provided. Often various strains of staphylococci and streptococci are causative microorganism; however, a wide variety of other anarobes microorganisms such as Baccteroides, Peptococcus, Peprtostreptococcus, Actinomyces, Eubacterium, and Fusohacterium, are sometimes found. These frequently encountered anaerobes are resistant to penicillin.
The clinical features:
On clinical examination the tooth with an acute abscess is painful to percussion, and if it is in occlusion, the patient complains that it seems “high” when it touches with the opposing tooth. As a rule it does not respond to electrical pulp tests. The application of ice, however, relieves the pain somewhat, in contrast to heat, which intensifies the pain. The tooth may display increased mobility.
If let to progress without treatment, the abscess may penetrate the jawbone at the thinnest and closest point to the root tip and form a space infection in the bordering soft tissues. The abscess area is painful, and the surface of the skin over the abscess feels warm and rubbery to palpation and shows fluild-like feel. The body temperature may be lifted. Aspiration usually produces yellowish pus. Regional lymph nodes may become enlarged and painful.
If circumstances are unfavorable, such as lowered host resistance combined with virulent multiplying organisms and inadequate early treatment, serious complications may occur. Complications like osteomyelitis, septicemia, septic emboli, asphyxia from a Ludwig’s angina or other space infection can compromise the airway and could be fatal.
A chronic infection happens when the virulence and number of the organisms are low and the host resistance is high If left untreated, the chronic abscess often forms a sinus tract, allowing the pus to drain to the surface. A small growth of inflammatory tissue forms on the surface and is called a parulis. When drainage is established, the tooth and associated swelling are no longer painful because the pressure of the abscess is lessened.
The differential diagnosis:
When a painful fluctuant swelling is present, the diagnosis of an abscess is suspected. Whether the abscess is the primary or secondary type, however, is more difficult to decide. This is because he original periapical lesion may not be easy to identify. Sometimes, this identification is often impossible because the tissue makeup has been destroyed by the infection.
If the abscess comes from a progression of pulpitis, cyst, granuloma, scar, or cholesteatoma is not of practical concern. However, it is critical when the abscess comes from either a secondarily infected primary tumor or secondary malignant tumor.
It is also necessary to consider that not all abscesses involving teeth are of pulpal origin. The periodontal (gum) abscess, originating in a deep periodontal pocket, is a common lesion and can be distinguished from the periapical abscess by proper radiographic examination. If the x-ray shows the absence of a periapical involvement, it usually is a periodontal abscess. In addition, the pulps of teeth with such periodontal abscesses are almost always, vital.
The recommeded treatment:
The acute abscess should be treated aggressively to alleviate the patients pain and to ensure that untoward sequelae do not occur. It is better to establish drainage immediately if possible, since this speeds the resolution of the abscess. Drainage may be set up by opening the pulp chamber and passing a file through the canal into the periapical region. When drainage cannot be established in this manner, a trephination procedure is suggested. This procedure involves making a window through the mucosa and bone to the abscess at the root tip. When the abscess spreads to the spaces around the chin, cheek, tongue and roof of the mouth, a through-and-through drain may need to be placed and frequently irrigated.
In more severe cases penicillin therapy should be began immediately, not less than 500 mg 4 times a day for at least 5 days. If the patient is allergic to penicillin, then our Houston dentist may prescribe erythromycin or clindamycin.
It is viewed unwise to extract a severely abscessed tooth (especially if much surgical manipulation is needed). Unless the patient has been adequately treated with antibiotics to ensure an effective blood level, the patient is at risk of the bacterial shower in the circulation produced by surgical manipulation in an abscessed area. Nevertheless, it is advantageous to keep the offending tooth once the acute phase of the infection has been controlled. Routine root canal treatment may be performed with or without a root resection to save the tooth.

i am 43 years old and suffering from NIDDM since the past 13 years and hypertension since the past 10 years.i am also suffering from angina and have undergone angiography twice.non dominant artery is completely blocked and even angioplasty cannot be done.i even have multiple slipped disks n spondilitisis.i even suffer from asthma.

the diabetes must be controlled. It is very dangerous if not controlled and can lead to many problems, it is probably one of the causes of your heart problems, in addition to lack of exercise, smoking, anger etc. You need to be treated well for this and advised well. For example, take your diabetes tablets even when you are not eating, etc. Your doctor should advice you on these things. Your doctor should have prescribed you a statin.
