For a fictional case. I have researched the Heart Assoc site and others, some things are unclear. A male in early 60s presents with chest pains, w/history of cholestorol and blood pressure. 1. Treatment: ABCDs: O2 (start with 2L nasal cannula with pulse oximetry to keep SpO2 > 95% + rest to give the potentially ischemic myocardium a break by increasing myocardial O2 delivery (DO2)), 325 mg ASA to chew, continuous 5-lead cardiac monitoring, IV saline locks X 2 (this is for safety + blood can be drawn from one of them — nitroglycerin (NTG) can cause potentially fatal complications (related to hypotension) in some “NTG-naive” individuals and complications in pt’s with aortic stenosis so you may need an IV fluid bolus handy), *then* sublingual nitro (0.4 mg spray or 0.3 mg tab, 1 every 5 min X 3 doses). If Trop I -ve, repeat in 6h; If +ve, overnight stay (on a cardiac monitor). Beta blocker. Morphine for pain + ongoing pain (PQRST) assessment and documentation (especially if titrating a NTG gtt).

Diagnose MI (STEMI vs NSTEMI) vs unstable angina vs other (i.e. non-cardiac) — all have different Txs. E.g. MI with ST-segment elevation (STEMI) is treated emergently with PCI or thrombolytics; NSTEMIs or STEMIs with no new LBBBs are treated c heparin or group IIb / IIIa antagonists (e.g. abciximab or other antiplatelets). ICU stay for unstable angina or discharge home if heartburn (“pink lady” or “white lightning” still given, but controversial in ruling out heartburn).

Ideally, ‘door to drug’ (i.e. TNK or tPA — “clotbusters”) or ‘street to stent’ for STEMIs should ideally occur within a 90 min. window; in reality though, most stents are placed electively in an outpatient setting (after the MI).

Do you know that Coronary Heart Disease is the most common type of heart disease in developed and industrialized countries? Coronary Heart Disease (CHD) is simply the disease of the coronary artery, a situation whereby the artery that supply blood to the heart is clogged or blocked, reducing blood flow to the heart muscles which need oxygen from the blood supply and this could lead to a complete artery blockage. When this happens in the heart, the result is heart attack medically known as myocardial infarction. The same process in blood vessels supplying the brain results in a stroke.
Coronary Heart Disease is caused mainly by:
formation of plagues (fatty deposits) in the lining of the arteries, rupture of this plagues and a subsequent development of a blood clot known as thrombosis in the artery.
These plagues narrow the arteries so that less blood flows through them thereby depriving the heart muscles of the much needed oxygen. If blood supply is not restored, there will be a permanent damage to the heart.

This is why Angina is typically triggered by physical activities. It can also occur after stress and anxiety.

Heart attack (myocardial infarction) occurs when the degree of narrowing of the coronary is high or if there has been a blood clot (thrombosis) which blocks the arteries. It is important to know that heart attack can occur anytime, anywhere without warning. These are the warning symptoms of heart attack: Chest discomfort, Sweating, Dizziness or nausea, Fatigue, palpitation, Fainting, breathlessness. Not all these symptoms occur in every attack

I’ve been having really bad heart pains like someone or something has been squeezing my heart. I went to a Cardiologist and he diagnosed it as “Muscular Discomfort”. It sounds about right but the pains have been occurring much more frequently. I looked it up and “Angina Pectoris” came up. I read the symptoms and it sounded right. Then I asked my mom and she brushed it off as “Anxiety” and “Panic Attacks”. But I’m not really worried about anything.

I went to the dr. on 1/31 with bronchitis like symptoms. He prescribed 7 days of antibiotics (Ketek) and albuterol breathing treatments. I used the nebulizer for the albuterol for about 4 days, then quit was having racing heartbeat, angina etc.

However I finished the Ketek. Two days ago I noticed a light rash on my chin, today the rash is on my entire face, neck, upper chest, back of neck and starting to spread across shoulder area. Very fine rash, not especially red or terribly itchy but there nonetheless.

I was told by my heart doctor that I have angina, but I feel that is not the whole answer to all my symptoms. Currently I constantly have chest pain/pressure, also occurs down my left arm, shoulders, neck, jaw, throat, back, stomach. I am a physician first year I wonder if the left-sided hypertrophy can cause angina pectoris (I think the left vent. hypertrophy causes compression in the system coronary vascular causes pain ischemic heart myocytes), but the breakdown on the left. hypertrophy is not mentioned as a factor predispose to angina pectoris in my book, I am a medical student, and is an academic question
